End-of-Semester Formative Feedback

Formal Student Evaluations of Teaching (SET) will not be collected for the Winter 2020 term in response to the disruption caused by the COVID-19 pandemic. Many instructors may still want to collect feedback to gather insight into their teaching and course design, and for inclusion in a teaching dossier. We have included three adaptable templates from which instructors can add or remove questions. We do ask that instructors remember to wait until the final grades have been submitted before requesting feedback from students. We are providing instructors with two options for getting feedback from students:

  1. instructors send a common anonymous link to their students in each class;
  2. instructors send a personal link to each student in each class.

Option A is the simplest to use, however do note that everyone who receives the email with the same, common link to the survey (i.e., not personal links there will not be any control over who may or may not complete the feedback form. Option B is more like the SET process, each student gets an individual form while remaining anonymous.

Option A:

  1. To begin, follow the instructions to create a feedback survey in Qualtrics
  2. Download the Qualtrics file (qsf file)
  3. Follow the instructions to create a common anonymous link to distribute the feedback form.

Option B:

  1. To begin, follow the instructions to create a feedback survey in Qualtrics
  2. Download the Qualtrics file (qsf file)
  3. Download your class list including email addresses and names using this download tool.
  4. Follow the instructions to import student contact email addresses for distribution.
  5. Instructions for Distribution from within Qualtrics.

While formal Student Evaluations of Teaching (SET) will not be collected for the Winter 2020 term in response to the disruption caused by the COVID-19 pandemic, some instructors may still wish to use the SET form to generate feedback. We have included here a Qualtrics version of the standard SET form for non-formal way of gathering summative feedback. Included in the Qualtrics survey are additional open-ended questions intended to provide formative feedback to the instructor. Follow the instructions for Option A or B when using this QSF file.
