How to Register a Grade 9 Student?
University of Windsor employees who wish to bring a grade 9 student to campus (either to participate in the planned group activities or to job shadow only) on November 6 must:
- complete the registration form
- submit the Waiver form signed by the student's parent or guardian
- submit a photo release form signed by the student's parent or guardian
Each University of Windsor employee can host a maximum of 2 grade 9 students.
As part of the registration process, a Waiver form must be completed and signed by the child's parent or guardian. You can download the Waiver in PDF fillable format here. You can upload the signed waiver to the online registration form, or e-mail it to Oliga Tserakhava at oligat@uwindsor.ca no later than October 23, 2024
Please note: photos will be taken during the day activities and may be posted on the University of Windsor social media channels. Hosts are asked to sign the photo release form. You can email it along with the signed waiver or bring with you on November 6.
Important: Please obtain prior approval from your supervisor for the job shadowing and also refer to the Health & Safety page for the list of areas across campus where grade 9 students are not permitted.
Please note that no registrations will be taken at the door.
