The sample below is of a Title page for a Master's Thesis in the Electrical Engineering program. Adjust the paragraph that begins with "A Thesis" based on your official department, program, and degree from the list of official Department and Program names and using the Title page Template to prepare your Title page.
Follow the instructions and populate your information in the Master's Committee page or Doctoral Committee page - below you can find snapshots with examples of how a completed Committee Members page should look:

Note that the Vita Auctoris is a required thesis element, however, there are no specific requirements or restrictions about its format or contents. It normally includes as a minimum:
- The author's name.
- Education and degrees.
For privacy concerns, do not include personal information such as home address and phone numbers, full date of birth, etc. Other information may be included that is directly related to the thesis or academic discipline (e.g., list of student’s publications/conference presentations resulting from their thesis research, etc.).
